For New Patients

My practice is full at the moment. PLEASE ONLY FILL OUT A CONSULT REQUEST if you have been referred by a clinician (a therapist, psychologist or doctor) and/or if you are pregnant, postpartum, or struggling with fertility issues.

As a patient, you can expect:

Quality Time
Quality health care requires quality time spent with patients. A new appointment with me will range from 60 to 75 minutes and cover an extensive medical and psychiatric history. Follow-up medication management appointments are 25 minutes in length and psychotherapy sessions are 45 minutes.

I provide my patients with access to my telephone number and will do my best to respond either the same day or within one business day. In the event of an emergency that requires an immediate response, please contact 911 or go the nearest emergency room after leaving me a message.

Unbiased Recommendations
Upon conducting an initial evaluation (or during the course of treatment), you will always be recommended the best treatment options, even if I do not provide the services myself and you have to be referred elsewhere.

Telephone Appointments
Occasionally, follow-up appointments may be done by telephone on a case-by-case basis. This is usually based on necessity, convenience, or other factors. These appointments may be shorter in duration but are still treated as appointments as far as billing is concerned.

Convenience of Payments
Fees are due at the time of the visit by cash or check. Exceptional arrangements are made on a case-by-case basis. The fee for the first visit is always due at the time of the visit without exception. Please contact me to discuss my fees for services.

Full Disclosure
I have no contractual obligations to any pharmaceutical companies.

Health Insurance Policy
Part of the reason I can afford to provide the services listed above is because I do not participate in any health insurance plans as an in-network provider, which allows me to see fewer patients, spend more time with each patient, and eliminate excess paperwork, thereby delivering what I consider to be higher quality health care. I am an out-of-network provider under all major insurance plans, and I will be happy to provide the necessary paperwork and documentation for patients to obtain reimbursement from their insurance companies.

Cancellation Policy
Appointment changes or cancellations must be made two business days prior, otherwise you will be billed for the full amount.

Don’t hesitate to contact me directly if you have any further questions.

Miriam Schultz, M.D.
